The preliminary investigation indicated that Chad Cook (26) of Adolphus was operating a 1996 Chevrolet pick-up that was pulling a bumper hitch trailer and was traveling southbound on US 31-E when it appeared that the trailer came loose and went into the northbound lane striking a 2001 Chevrolet pickup that was traveling northbound and being operated by Steven Herrington (51) of Scottsville. The collision caused Herrington’s truck to spin out of control and strike two Kentucky Transportation Cabinet Highway Department workers that were on the Bays Creek Bridge conducting maintenance. The trailer then struck another northbound vehicle, a 2014 GMC pick-up, that was being operated by Garry Wade (57) of Scottsville.
Mr. Cook and his three passengers, along with Mr. Herrington, were not injured in the collision. Mr. Wade was transported by private means to the Medical Center in Scottsville where he was treated for non-life threatening injuries.
Jeffrey Harrison (56) of Scottsville was one of the workers on the bridge and he was transported by the Allen County EMS to the Medical Center in Bowling Green for non-life threatening injuries.
The other worker on the bridge that was struck, Titus Morris (34) of Scottsville, was transported to the Medical Center in Scottsville where he was pronounced deceased by the Allen County Coroner's Office.
The investigation is still ongoing and being led by Trooper First Class Brian Gann. He was assisted on the scene by Master Trooper Forrest Winchester, the Allen County EMS, Allen County Coroner's Office, Allen County Sheriff's Department, Scottsville Police Department, and the Scottsville Fire Department.
